QuickBase includes a lot of detail in its URLs, which you can take advantage of when building formulas or external API functionality. For example, let’s consider a URL a user might see when viewing a report:

Typically in QuickBase, forms usage is determined at either the role level, or at the report level. You can, however, use a formula URL button to call upon specific forms regardless of forms usage settings. This is useful if you‘ve created multiple data entry forms, each with their own layout, that you wish to call upon at various stages of the record entry process.

That field must appear more than once on your form. When that's the case, QuickBase must choose which value to enter in the field. To break the tie, QuickBase always takes the value from the last instance of the field, which in your case is the one you're leaving blank.

If the duplicate field is a mistake, just delete the double from your form. The field will then save values as you expect.
